The Revolution PHR (Personal Health Record) is a secure, online personal health record that we provide for our patients that allows them to: Update personal information, such as phone number or address changes View medical history Review account balances Book and view upcoming appointments online View prescriptions for contacts and glasses and check the status of these orders Communicate essential information directly with your doctor View/update all of the above for other family members (must be granted access)
